SDPW- The Conference.
No part of any form should be modified or
reproduced in mass quantities unless approved by SDPW.
**The
*2008 Physical must be turned in with an original stamp or signature
from a Physician.
*No photocopies of the Physicians
signature or stamp will be accepted.
*All
Waivers must be signed by the boundary association to participate in
2008.
*SDPW
or National Pop Warner does not give waivers for age or weights.
*All
Volunteers must have a background check before receiving a badge. |
